Please read carefully.
BY USING THIS WEBSITE, YOU AGREE TO THESE TERMS AND CONDITIONS.
This policy explains how the Sourcewell handles information collected from you during your visit to the website. Sourcewell may amend this policy on occasion and will prominently display on this website any major changes in the way it uses your personal information. During use of the website certain information is collected automatically.
All public and personal information sent to or collected during your access to or use of this website is governed by the Minnesota Government Data Practice Act (MGDPA) at Minnesota Statutes Chapter 13, and Sourcewell’s data practices policy. Under the MGDPA, some information may be subject to inspection by or disclosure to the public. This Policy explains how Sourcewell complies with these requirements.
Information Collected and Stored Automatically:
Sourcewell uses Google Analytics and other tools to automatically collect and store information about your visit to this website for statistical analysis and promotional purposes. These tools do not collect personal information. Rather, the tools collect and store information such as: (1) your Internet Protocol Address (IP Address), geographic location of your IP Address, and domain name; (2) the type of browser and operating system you used; (3) the date and time you visited this site; (4) the web pages or services you accessed at this site; (5) the website you visited prior to coming to this website; and (6) generalized demographic information that does not identify individuals.
If, during your visit, you participate in a survey, fill out a web form, send an email, or submit a comment, the following additional information will also be collected: (1) your email address and the contents of the email; and (2) any information volunteered to complete a web form, respond to a survey, or submit a comment, which may include audio, video, and graphic information formats you send to us. This information is retained in accordance with Minnesota Statutes, sections 138.17, subd. 1 and 15.17.
Personal Information and Choice:
Personal information is information about a natural person that is readily identifiable to that specific individual. Personal information may include the individual’s name, address, phone number and email address. A domain name or IP Address is not considered personal information.
Sourcewell will not request personal information about you unless you voluntarily participate in an activity that asks for the information, such as completing a survey or web form, sending an email, or submitting comment. Sourcewell will only use this information to respond to you or provide information and updates relevant to your request and interest in Sourcewell. Sourcewell will not disclose, give, sell, or transfer any personal information about website visitors. However, this information is a public record once you provide it, and it may be subject to public inspection and copying under the MGDPA.
Sourcewell may share any public data sent to or collected from the site with other entities as permitted under the MGDPA.
Access and Correction of Personal Information:
You may review any personal information we collect about you. You may recommend changes to your personal information you believe in error by submitting a written request that credibly shows the error. If you believe that your personal information is being used for a purpose other than what was intended when you submitted it, contact us at firstname.lastname@example.org. In all cases, we will take reasonable steps to verify your identity before granting access or making corrections.
Sourcewell uses “cookies” to customize your browsing experience, enhance site navigation and experience, analyze site performance, and monitor usage.
What are cookies?
Whenever you visit a website a small piece of information called a cookie is sent along with the webpage. Cookies contain unique information to track such things as the geographic location of your IP address, the type of browser and operating system you used, the date and time you visited this site, and the website from which you linked to this website. (For more information on what information Sourcewell collects, please reference the Personal Information and Choice section above.)
There are two kinds of cookies. A session cookie is a line of text that is stored temporarily in your computer’s random access memory (RAM). A session cookie is never written to a drive, and it is destroyed as soon as you close your browser. A persistent cookie from a website is saved to a file on your hard drive and is called up the next time you visit that website. This lets the website remember what you were interested in the last time you visited.
The site creates a session cookie when you visit and destroys it as soon as you close your browser. Google Analytics and other analysis and performance tools configured on sourcewell-mn.gov use first-party (persistent) cookies to: (1) distinguish unique users; (2) throttle the request rate.
Although using persistent cookies allows us to deliver a better experience for you, this site will work without them. If you wish to delete a cookie file, please use your browser’s setting or any of the widely available methods; if you don’t want to accept cookies in the future, you may use your browser's options to set your preferences.
For site security purposes and to ensure that this service remains available to all users, Sourcewell use industry-standard methods to monitor network traffic to identify unauthorized attempts to upload, or change information, or otherwise cause damage. Unauthorized attempts to upload or change information on Sourcewell servers are strictly prohibited and may be punishable by law.

Any commercial use, reproduction for purposes other than described above, modification, distribution, republication, display, or performance is strictly prohibited. The submission of any materials to Sourcewell irrevocably waives any and all ‘moral rights’ in such materials, including the rights of paternity and integrity.
